If you want to just ask for admins to get on to help with a player, post it in the chatbox or join our TS and poke an Admin. If you want to make a full-fledged report on someone, you're going to need to follow the format and provide evidence.

I believe I banned him once. I forgot, I think you were on when I banned him. I was spectating him. (the ban was long ago btw)
