You should include your Steam ID, since you left that blank.
Also, you aren't a donator, since you never even applied for Member.
If you pay the $2 fee, you should get unbanned, just make sure to include your Steam ID so we can actually complete the unbanning process. If you do not know how to do so, follow these instructions:

1. Go to this website.
   
2. Copy and paste your steam profile URL into the input and click the button.
   
3. Copy and paste the "steamID" part.
