Requirments: 
App name must be "[Server name]Name's Admin|Operator Template"(you need to choose either admin or operator, and [Server name] would be replaced with [TTT], [Sandbox], or whatever the name of the server you are applying for is)
You must have a vouch from an Admin or Operator to get Operator.
Time requirments are absolute, whether you get a vouch or not.
Replace "[answer]" with your answer
Remember, getting a promotion grants you a lot of power and with that power come heavy responsibilities.

Half of this information is useless. 
This is an English server so why the fuck should we be multilingual? 
We dont need them to repost their steamid because it's added to the database as soon as you become a Member. 
We can also look up playtimes ourselves and deny people who don't meet them. 
Steam profile = steamid, see response about that.
Age is on your Member application, we don't need the same information twice.
Requiring people to buy a mic isn't fair on them cus maybe they just don't want to talk?
Experience with ULX is not necessary, because it's not super complicated to learn it. You open the menu and select a command.
The only thing I would support is putting (name)'s (server) (op/admin) Application. Everything else is useless information.

@"『TXM』Xenon" I can see the direction that you're coming from. Although, throwing up red tape and bureaucracy can potentially lead to more issues. Keeping things simple keeps everything running smoothly. For example, a bunch of people can't even fill out the current form, how will they accurately fill out a longer (more complex one). Unless additional software is tailored such as email me forms.

With more fields, there's additional needless information for @SaintLuther to review. Total waste of time if the application is denied anyways.

1. This is a terrible suggestion in my opinion, i mean why make it more complicated when it works just fine as it is.

2. As @Zellogi said, most of this stuff is either useless or unnecessary.

3. Why would you force people to get mics if they want to be staff, there are plenty of good admins/operators that do a good job without using a mic.

4. I only support the app name suggestion (saying what server you are applying for on the threads name) but this won't make much of a difference anyways.
